Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years of direct patient care, including 2 years in hospice/home-based care.
• Demonstrated ability to apply hospice principles and regulatory compliance (Medicare/Medicaid/JCAHO/ACHC).
• Knowledge and compassion for terminally ill patients and families.
• Ability to lead a diverse care team with strong interpersonal skills.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritization based on clinical judgment.
• Triage incoming referrals and medical concerns promptly and compassionately.
• Lead IDG meetings and document minutes.
• Mentor and support associates in development and compliance.
• Ensure care delivery meets regulatory standards and company policies.
• Support performance improvement and operational efficiency at the location.
