Advance Hospice Care is seeking a Patient Care Manager to lead clinical operations and ensure compliant, high-quality hospice services. You will oversee patient care delivery, coordinate visits, and mentor staff while maintaining regulatory standards.

Qualifications

Current RN license in the state of employment.
Graduate of an accredited nursing program.
Minimum of 3 years of direct patient care experience, including hospice or home-based care.
Knowledge of hospice principles and regulatory requirements (Medicare, Medicaid, JCAHO, ACHC).
Ability to lead a diverse care team and handle emotional/clinical patient needs.

Responsibilities

Oversee and direct patient care services to ensure high-quality hospice care.
Manage visit scheduling and prioritization based on clinical judgment.
Triage referrals and patient concerns promptly and compassionately.
Lead or facilitate IDG meetings and coordinate care plans.
Mentor and train associates to support development and compliance.
Ensure compliance with federal, state, and local standards and company policies.
Support performance improvement initiatives, quality assessments, and operational efficiencies.
