Job summary

Advance Hospice Care is seeking a dedicated Patient Care Manager RN to lead clinical operations at our Alabama location. You will oversee patient care delivery, schedule visits, triage referrals, and mentor staff to ensure compliant, high-quality hospice services.

You will lead IDG meetings, coordinate care plans, and ensure adherence to Medicare/Medicaid and JCAHO/ACHC standards while supporting performance improvement and operational efficiency.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care, including hospice/home health.
• Knowledge of Medicare/Medicaid, JCAHO, ACHC regulations.
• Experience leading interdisciplinary teams.
• Ability to mentor staff while ensuring regulatory compliance.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritization based on clinical judgment.
• Triage referrals and medical concerns promptly and professionally.
