The role reports to the Executive Director and emphasizes regulatory compliance, performance improvement, and coordinating an Interdisciplinary Group to optimize patient outcomes. RN license required.

Qualifications

• Current RN license in Indiana
• Minimum of 3 years direct patient care, including 2 years in hospice or home health
• Demonstrated ability to apply hospice principles, regulatory standards (Medicare/Medicaid/JCAHO/ACHC)
• Strong leadership and mentoring skills
• Competency in clinical documentation and technology systems

Responsibilities

• Oversee and direct patient care services to ensure quality hospice care
• Manage visit scheduling and prioritization based on clinical judgment
• Triage incoming referrals and medical concerns promptly and compassionately
• Lead Interdisciplinary Group (IDG) meetings and document minutes
• Mentor, train and support staff to ensure compliance and performance
• Ensure care delivery meets regulatory and company standards
• Support performance improvement initiatives and operational efficiencies
