Gentiva is seeking a Patient Care Manager to lead hospice patient care operations and ensure regulatory compliance. You will oversee scheduling, triage referrals, and coordinate interdisciplinary care plans to deliver compassionate, high-quality service.

Ideal candidates hold an RN license with hospice experience, optional BSN, and CHPN certification. This role emphasizes leadership, team mentoring, and adherence to standards across multiple care settings.

Qualifications

• RN license in the state of employment, with a nursing degree from an accredited program.
• Minimum 3 years of direct patient care, including 2 years in hospice or home-based care.
• Ability to apply hospice principles and regulatory standards (Medicare/Medicaid/JCAHO/ACHC).

Responsibilities

• Oversee and direct patient care to ensure high-quality hospice services.
• Manage visit scheduling and prioritize clinical decisions.
• Triage referrals and medical concerns promptly and compassionately.
• Lead IDG meetings, present updates, and document minutes.
• Mentor and train associates; ensure compliance and performance.
• Ensure care delivery meets federal, state and local standards.
