Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care experience, including 2 years in hospice or home-based care.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritize based on clinical judgment.
• Triage incoming referrals and medical concerns promptly and compassionately.
• Lead IDG meetings, present patient updates, review charts, coordinate care plans, document minutes.
• Mentor and support associates, promoting development, performance and compliance.
• Ensure care delivery meets regulatory standards and company policies.
