Job summary

Community Healthcare of Texas is seeking a Patient Care Manager in Waco, Texas to provide clinical oversight of direct patient care and lead a team of hospice professionals.

This role emphasizes coordination with physicians, case conferences, staff hiring and training, and ensuring compliance with regulatory standards.

The ideal candidate will have an RN license in Texas, a BSN, at least two years in hospice, and strong leadership and communication skills.

Qualifications

• Requires college degree in nursing; BSN strongly preferred.
• 2+ years hospice experience and supervisory experience.
• Licensed RN in Texas.
• Valid TX Driver's License.

Responsibilities

• Oversee the admission process for new patients and coordinate with the team and hospice physician as needed.
• Assign staff to patients and schedule for budget optimization, including PTO and on-call.
• Review patients' medical diagnoses, prognosis, medications, and care plans.
• Assist in establishing goals, priorities, and plans of care with clinicians.
• Attend case conferences to coordinate interdisciplinary care.
• Assist in interviewing and orienting new staff; support ongoing staff education.
• Assist in planning, implementing, and evaluating education programs and policies.
• Audit patient charts and ensure regulatory compliance; write QA reports.
• Lead hiring, performance evaluation, coaching, scheduling, and delegation.
