Community Healthcare of Texas is seeking a Patient Care Manager in Waco, TX to oversee direct patient care, assign staff, coordinate interdisciplinary teams, and ensure care meets the mission and service standards.

The role requires a nursing degree, Texas RN license, hospice experience, supervisory experience, and strong communication; travel to support clinical operations and staff development is expected.

Qualifications

• Requires a college degree in nursing; BSN strongly preferred.
• Requires 2+ years of hospice experience and supervisory experience.
• Experience in a Charge Nurse role will be considered to meet the supervisory requirement.
• Hospice experience and knowledge of hospice regulations strongly preferred.
• Proficient skills using a computer; Microsoft Office suite experience is preferred.
• Good communication skills and the ability to travel independently.

Responsabilités

• Oversee the admission process for new patients and coordinate with the team and hospice physician as needed.
• Manage direct patient care by assigning staff, scheduling, and addressing care issues.
• Review patients' diagnoses, prognosis, medications, and clinical course.
• Assist in establishing goals and plans of care with clinicians.
• Attend case conferences to coordinate care and interdisciplinary involvement.
• Educate, counsel, and orient staff to agency policies and procedures.
