Job summary

Community Healthcare of Texas is seeking an experienced RN Case Manager to coordinate hospice services, meet with patients and families, and ensure a comprehensive plan of care. You will lead nursing assessments, modify care plans, and work with the physician and interdisciplinary team to optimize patient outcomes.

The role requires Texas RN licensure, a valid driver's license, and at least 1 year of nursing experience (hospice experience preferred).

Qualifications

• Licensed Registered Nurse in Texas with an active license.
• Valid driver’s license and reliable transportation.
• 1 year of nursing experience; hospice case management experience preferred.
• Proficient computer skills; MS Office experience preferred.

Responsibilities

• Meet with patients and families to explain hospice services and coordinate admissions.
• Develop and adjust care plans based on nursing assessment and patient needs.
• Administer medications and treatments as prescribed by the physician.
• Coordinate patient care with physicians and the hospice interdisciplinary team.
• Participate in on-call rotation and assist with admits.
