Job summary

Geiss Med Hospice in Idaho seeks an experienced Hospice Account Executive to manage and strengthen relationships with referral partners, coordinating care transitions and ensuring seamless continuity for patients entering hospice services.

You will collaborate with discharge planners, clinicians, and sales partners, educate referral sources on eligibility and benefits, and represent Geiss Med Hospice at community events while upholding compassionate, patient-centered care.

Qualifications

• Experience in hospice, home health, or healthcare account management preferred.
• Strong communication and relationship-building abilities.
• Ability to work independently and collaboratively in a multidisciplinary team.

Responsibilities

• Develop, manage, and maintain relationships with referral partners including hospitals and clinics.
• Serve as the primary contact for referral sources with consistent communication.
• Collaborate with discharge planners, case managers, social workers, and clinical teams for smooth patient transitions.
• Monitor referral processes to align patient needs with care plans.
• Provide ongoing education to referral partners about hospice eligibility, services, and care coordination.
• Support referral sources by addressing questions and operational challenges.
• Work with the interdisciplinary hospice team to align patient care goals and improve workflows.
• Participate in team meetings, care coordination discussions, and performance reviews.
• Identify opportunities to improve communication and partner satisfaction.
• Represent Geiss Med Hospice at community meetings and professional events.
