Job summary

Affinity Health Management is seeking a Nurse Practitioner to provide comprehensive hospice services under physician supervision in home and community settings. The role includes evaluating patient health histories, diagnosing conditions, directing care plans, and coordinating with the hospice interdisciplinary team to meet patient needs.

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current DEA license.
• Current nursing licensure in the state (Ohio) and CPR certification.
• Master's degree with at least 1 year NP experience and 1 year hospice/palliative care experience.
• Certification from an approved certifying body for advanced practice nursing (preferred; CHPN, etc.).

Responsibilities

• Provide medical care and manage terminal illness under physician supervision.
• Complete history and physical exams, order/interpret tests, document findings.
• Develop an individualized plan of care, prescribe medications, coordinate hospice services.
• Complete hospice face-to-face visits for eligibility and recertification, with medical director input.
• Coordinate with hospice interdisciplinary team to ensure symptom control and care continuity.
• Educate patients and families about care plans and end-of-life issues.
• Establish and maintain relationships with staff and community referral sources.
