Job summary

Affinity Health Management in Warren, Ohio seeks a licensed Nurse Practitioner to provide comprehensive hospice services under physician supervision. You will evaluate health histories, diagnose conditions, develop and direct the plan of care, and coordinate hospice services in patients’ homes and communities.

The role requires current state licensure, a Master's degree and NP certification, DEA license, and hospice or palliative care experience.

Qualifications

Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
Current DEA license.
Current nursing licensure in state and CPR certification.
Master's degree with a minimum of one year Nurse Practitioner experience and a minimum of one year hospice or palliative care experience.
Accreditation from an approved certifying body for advanced practice nursing as required by State.
Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing).

Responsibilities

Provides medical care, palliates and manages terminal illness under supervision of a collaborating physician.
Completes history and physical examinations, orders and interprets tests, documents findings.
Develops individualized plan of care, prescribes medications/treatments, coordinates hospice care services.
Completes hospice face to face visit requirements to assess eligibility.
Reviews and coordinates patient care with hospice interdisciplinary team.
Educates patients and families on preventive care and disease management.
Maintains safe environment and continuity of care with family/caregiver involvement.
