Job summary
Affinity Health Management is seeking a Nurse Practitioner to provide comprehensive hospice services under physician supervision. The NP will evaluate health histories, diagnose conditions, direct care plans, prescribe treatments, and coordinate with patients, families, and the hospice interdisciplinary team in home and community settings.
The role requires an NP with a Master's in Nursing, Ohio licensure, and DEA certification, plus hospice or palliative care experience.
Qualifications
- Master's degree in Nursing and NP credentialing required.
- DEA license and current Ohio nursing license are mandatory.
- Hospice or palliative care experience preferred.
- Strong assessment, documentation and care-planning capabilities.
- Excellent communication with patients, families and interdisciplinary team.
Responsibilities
- Provide medical care for terminally ill patients under physician supervision.
- Complete histories and physical exams, order and interpret tests.
- Develop and manage individualized hospice care plans and med/treatment orders.
- Report hospice eligibility in line with participation rules and determinations.
- Coordinate and communicate with interdisciplinary team and family.
- Maintain accurate clinical notes and documentation.
- Educate patients/families on care, risks, and end-of-life options.
- Ensure safety and continuity of care for patients and caregivers.
