Qualifications

• Master of Science in Nursing (MSN) or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Current RN and NP licensure in state of practice.
• Active NP certification (ANCC or AANP).
• DEA registration and prescriptive authority where applicable.

Responsabilités

• Provide patient-centered clinical care in hospice and palliative settings.
• Perform F2F visits for eligibility and recertification per CMS timelines.
• Collaborate with physicians and IDG/IDT to coordinate Plan of Care.
• Prescribe medications within scope of practice and CPPA agreements.
• Coordinate care across home, SNF, and assisted living environments.
