Job summary

AdvisaCare Hospice and Palliative Care Program is seeking a Nurse Practitioner to join our team in Michigan. The NP will perform home-based evaluations, manage medications, and coordinate with the interdisciplinary team to support patient goals and comfort.

Responsibilities include medical assessments, ordering tests, and maintaining timely documentation while collaborating with the Medical Director. Ideal candidates bring hospice/home health experience and a strong patient-centered approach.

Qualifications

• Must be a current unrestricted Nurse Practitioner licensed in Michigan.
• One-year hospice/home health experience preferred.
• Knowledge of home health, hospice, or outpatient services preferred.
• Willing to support Hospice Medical Director/designee.
• Commitment to confidentiality and professional relationships.

Responsibilities

• Performs routine and urgent assessments in patients' homes.
• Conducts medical evaluations of elderly, frail patients.
• Orders and interprets labs and radiology tests within scope.
• Prescribes medications as delegated and licensed.
• Ensures accurate documentation and care plans at every visit.
• Coordinates with the Interdisciplinary Team and Medical Director.
