Job summary

Gentiva seeks a Nurse Practitioner to deliver comprehensive hospice and palliative care, performing patient management visits and hospice Face-to-Face encounters for eligibility and recertification. You will work closely with physicians and interdisciplinary teams to ensure high-quality, compliant, compassionate care.

You will manage assessments, symptom control, and end-of-life discussions, coordinating care across home and facility settings while adhering to Medicare/Medicaid requirements.

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years NP experience preferred.
• Experience in hospice, palliative care, oncology, geriatrics, family/adult medicine, home health, or acute care preferred.

Responsibilities

• Provide comprehensive, patient-centered hospice and palliative care.
• Perform F2F visits for eligibility certification/recertification per CMS timelines.
• Collaborate with IDG/IDT and PCPs to coordinate care.
• Prescribe medications within state regulations and CPP agreements.
