Mid Michigan Home Health and Hospice is seeking a compassionate Hospice Nurse Practitioner to join our interdisciplinary team. You will provide high-quality palliative and end-of-life care to patients in their homes and other settings, focusing on comfort and dignity.

You will collaborate with physicians, nurses, social workers and spiritual care staff; educate families; manage symptom control; and document in EMR while participating in on-call rotations.

Formación

• Current and unrestricted NP license in the state.
• Active or eligible for DEA license.
• Minimum 1 year hospice/palliative or geriatrics experience preferred.
• Strong clinical assessment & communication skills.
• Valid driver’s license and reliable transportation.

Responsabilidades

• Provide high-quality palliative and end-of-life care in patients' homes and care settings.
• Develop and manage individualized hospice care plans.
• Collaborate with physicians, nurses, social workers, and spiritual care staff.
• Educate patients and families on disease progression and comfort measures.
• Document encounters in EMR and participate in on-call rotations.
