Heartland Hospice is seeking a Nurse Practitioner to provide dual hospice and palliative care, delivering patient-centered management across serious illness. You will conduct F2F eligibility visits, collaborate with IDG/IDT, and guide patients and families through complex decisions and end-of-life care.

The role requires MSN/NP credentials, experience in hospice/palliative care, and ability to work autonomously within a team. Competitive benefits and full-time schedule offered.

Qualifications

• Master of Science in Nursing (MSN) from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years NP experience preferred.
• Experience in hospice, palliative care, oncology, geriatrics, home health, or acute care preferred.
• Experience with Part B billing preferred.

Responsibilities

• Complete all visit types for hospice and palliative patients including F2F visits within CMS timeframes.
• Collaborate with Hospice and Palliative Medical Directors and IDG/IDT to deliver care.
• Coordinate patient Plan of Care with PCP/Attending Providers.
• Perform comprehensive patient assessments and manage treatment plans.
• Prescribe medications within scope and regulatory parameters.
• Order and interpret labs and tests per state scope.
• Facilitate goals-of-care discussions and advance care planning.
• Coordinate care across home, SNF, and assisted living settings.
• Maintain compliant clinical documentation.
