Job summary

VIA Health Partners is seeking a Nurse Practitioner to provide comprehensive end-of-life and palliative care. The role focuses on symptom management, patient and family counseling, and coordination with medical staff to develop care plans.

Responsibilities include prescribing therapies, conducting care discussions, and ensuring compliant documentation. This is a full-time position at our Charlotte, NC location with a patient-centered mission.

Qualifications

• Master's degree in nursing and active NC RN license.
• Nurse Practitioner certification from NC Board of Nursing.
• Board certification within 2 years of hire.
• DEA license and Basic Life Support (BLS) required.
• Computer proficiency is required.

Responsibilities

• Promotes high quality end-of-life care and use of advance directives.
• Diagnoses, treats, and manages pain and other symptoms.
• Prescribes therapies and medications per medical ethics.
• Counsels patients and families on prognosis and goals of care.
• Performs patient/family meetings to discuss care plans.
• Submits billing and documentation according to policy.
