Job summary

Heartland Hospice is seeking a Nurse Practitioner to provide comprehensive hospice and palliative care. The role includes patient management visits, hospice Face-to-Face encounters for eligibility, and collaboration with physicians and IDG/IDT to ensure compliant, compassionate care.

The position emphasizes autonomous practice within state regulations, with emphasis on symptom management, care planning, and coordination across settings to optimize quality of life for patients and families.

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years Nurse Practitioner experience preferred.
• Experience in hospice, palliative care, oncology, geriatrics, family/adult medicine, home health, or acute care preferred.
• Experience with Part B billing preferred.

Responsibilities

• Complete all visit types for hospice/palliative care patients, including CMS Face-to-Face visits within regulatory timeframes.
• Collaborate with Hospice & Palliative Medical Directors and IDG/IDT to deliver patient-centered care.
• Coordinate with PCP/Attending Providers and specialists for Plan of Care.
• Perform in-depth patient assessments across physical, emotional, functional, and psychosocial domains.
• Manage and optimize treatment plans, symptoms, and end-of-life care within scope of practice.
