Job summary

Heart 'n Home Hospice, part of the LHC Group within the Optum family, seeks a Hospice Nurse Practitioner to provide palliative and supportive care in a patient-centered setting. You will collaborate with the Hospice Medical Director, coordinate plans of care, and guide patients and families through complex symptom management.

Qualifications

• Active unrestricted license in the state of residence
• Current certification as a clinical Nurse Specialist or Nurse Practitioner
• Current CPR certification
• 5+ years of nursing experience
• Current driver's license and vehicle insurance

Responsibilities

• Makes face-to-face encounter visits within prescribed time limits to Hospice patients
• Reports clinical findings to the Hospice Medical Director
• Informs primary physician of recommended services and collaborates with other physicians
• Initiates and coordinates the plan of care with patient/family and IDG
• Maintains accurate clinical notes and communicates patient needs to Administrator and Hospice Medical Director
