Job summary

Heartland Hospice is seeking a licensed Nurse Practitioner to serve in dual hospice and palliative care roles. You will deliver advanced clinical care, perform patient management visits and hospice Face-to-Face visits for eligibility and recertification, collaborating with physicians and IDGs to ensure high-quality care.

The role blends autonomy with teamwork, focusing on symptom management, end-of-life care, and patient-family support across care settings.

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited NP program.
• Minimum of two years NP experience preferred.

Responsibilities

• Perform patient management visits and hospice F2F encounters for eligibility and recertification.
• Collaborate with IDG/IDT to ensure quality, compliant care.
• Coordinate care across settings including home and SNF.
• Prescribe meds and order/interpret tests within scope of practice.
• Maintain compliant documentation and meet regulatory standards.
