Job summary

Qualifications

• RN license in Tennessee and APN license (APRNs) required.
• Baccalaureate Nursing degree from an accredited program.
• Two years of APN experience in clinical care, hospice or home health preferred.
• Valid driver's license and reliable transportation; auto liability insurance required.
• Comprehensive knowledge of pharmacology, palliative care, and end-of-life care.

Responsibilities

• Consult with patients and families about disease process and end-of-life care.
• Assist in IDG development of the patient/family plan of care.
• Collaborate with hospice physician and other clinicians.
• Inform team of changes in patient needs and outcomes.
• Ensure timely documentation and compliance with regulations.
• Perform other duties as required.
• Maintain compliance with laws and policies.
