Job summary

Providence at Home with Compassus is seeking a Hospice and Palliative Care Nurse Practitioner to function as an independent advanced practice provider. You will assess patients, develop plans of care, and coordinate with the interdisciplinary team, including end‑of‑life visits and recertifications, under physician orders or approved protocols.

Responsibilities include prescribing medications, participating in on‑call rotations, educating patients and families, and contributing to quality

Qualifications

• Master’s Degree in Nursing is required.
• Two years of hospice, acute care, or transitional care experience required.
• DNP preferred.
• Active NP license and RN license required.

Responsibilities

• Comply with policies, regulatory requirements and safety standards.
• Perform comprehensive medical, functional, and psychosocial assessments.
• Independently prescribe/adjust medications per protocols.
• Conduct provider face-to-face, end-of-life, and recertification visits.
• Develop and revise plans of care with the IDG; monitor care.
• Educate patients, families, and staff on diagnoses and prognosis.
• Assist families with decision-making on interventions and end-of-life planning.
