Job summary

Heartland Hospice is seeking a Nurse Practitioner to serve in a dual hospice and palliative care role in the United States. You will provide comprehensive, patient-centred care across the continuum of serious illness, supporting patients in both palliative and hospice settings.

The role blends autonomy with collaboration and requires adherence to regulatory standards. You will deliver advanced clinical care, perform patient management visits and hospice Face-to-Face (F2F) encounters for

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years of Nurse Practitioner experience preferred.
• Hospice/palliative care experience preferred.
• Part B billing experience preferred.

Responsibilities

• Provide advanced clinical care and F2F visits for eligibility and recertification.
• Collaborate with IDG/IDT to develop and adjust Plan of Care.
• Prescribe medications and treatments within state and regulatory scope.
• Document findings accurately and maintain regulatory-compliant records.
• Coordinate care across home, SNF, and assisted living settings.
