Job summary

Gentiva is seeking a Nurse Practitioner to deliver dual Hospice and Palliative Care, providing comprehensive care across serious illness. You will perform management visits, F2F for eligibility, and collaborate with physicians and the interdisciplinary team to ensure high-quality, compliant care for patients and families.

You will balance autonomy with teamwork, guiding complex medical decisions, symptom management, and end-of-life care while maintaining regulatory standards and patient-centered

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Two years NP experience preferred.
• Experience in hospice/palliative care preferred.
• Experience with Part B billing preferred.

Responsibilities

• Complete hospice/palliative visit types per CMS timeframes.
• Collaborate with IDG/IDT to deliver patient-centered care.
• Coordinate with PCP and specialists to plan care.
• Perform in-depth patient assessments across domains.
• Prescribe and manage medications within scope.
• Order and interpret labs within scope of practice.
• Facilitate goals-of-care discussions and care transitions.
• Maintain timely, compliant clinical documentation.
