Job summary

UnitedHealth Group is seeking a Hospice Nurse Practitioner to provide consultation in palliative and supportive care for hospice patients, coordinating plans of care with physicians and families. The role involves direct patient encounters, collaborative decision-making, and documentation of care plans.

The position emphasizes interdisciplinary teamwork, excellent clinical judgment, and compassionate patient-family communication.

Qualifications

• Active unrestricted nursing license in the state of practice.
• Current certification as a clinical Nurse Specialist or Nurse Practitioner.
• Current certification by a national body (ANCC, NBCHPN, or AANP).
• Current CPR certification.
• 5+ years of nursing experience.
• Valid driver's license and access to a vehicle or reliable transportation.

Responsibilities

• Conduct face-to-face hospice visits within prescribed time limits.
• Consult with Hospice Medical Director and collaborating physicians.
• Initiate and coordinate plans of care with patient/family input.
• Educate patient/family on self-care techniques when appropriate.
• Maintain accurate clinical notes and report changes to leadership.
