Job summary

Harmony Hospice seeks a Nurse Practitioner to join our team in a full-time capacity. You will provide advanced clinical care, symptom management, and collaborative medical leadership to ensure exceptional end-of-life care for patients and support for families.

Responsibilities include patient visits in multiple care settings, pain and symptom management, and close collaboration with physicians and the interdisciplinary team.

Qualifications

• Graduate of an accredited School of Nursing and accredited NP program.
• Current, active NP license in Rhode Island.
• Current DEA license.
• Current BLS/CPR certification.
• Master's degree required.
• Minimum 1 year NP experience.
• Minimum 1 year hospice/palliative care experience.
• Certification from an approved APN certifying body; specialty cert preferred.
• Valid driver's license with reliable transportation.

Responsibilities

• Provide advanced medical care and clinical support to hospice patients and families.
• Evaluate, diagnose, and manage pain, symptoms, and terminal illnesses per best practices.
• Perform comprehensive patient assessments and determine ongoing eligibility/prognosis.
• Conduct patient visits in homes, SNFs, ALFs, hospitals, and via telehealth when appropriate.
• Collaborate with physicians and interdisciplinary team to plan and coordinate care.
• Communicate with referral sources regarding new patient referrals and continuity of care.
• Complete face-to-face encounters and maintain timely clinical records per regulations.
• Provide education and clinical guidance to patients, families, caregivers, and staff.
