Job summary

Presbyterian Healthcare Services seeks a part-time Clinical Coordinator to facilitate hospice and palliative care referrals. You will evaluate complex medical data to correctly match patients with the appropriate program and level of care, bridging gaps between referral sources and end-of-life services.

The role emphasizes advanced bedside assessments, symptom management, and primary hospice RN responsibilities within hospital settings.

Qualifications

• Associate's degree in nursing is required.
• New Mexico RN License is required.
• 1 year of hospice experience is required.
• 1 year of acute care experience is strongly preferred.
• Current driver’s license and auto insurance are required.
• BLS certification is required.

Responsibilities

• Bridge referrals from sources to specialized end-of-life care and evaluate data to match patients to the correct Hospice or Palliative Care program and level of care.
• Perform advanced assessments evaluating physical, psychological/social, functional, financial, resource and spiritual elements.
• Support hospital rounding as the primary hospice RN and direct the initial plan of care.
• Serve as senior consultant for conversations about goals of care, hospice criteria and preferred sites of care.
• Review referrals for clinical suitability and assign cases based on patient needs and team capacity.
• Provide subject matter expertise across the transitions service line for multidisciplinary team members.
