Job summary

Concho Hearts Hospice is seeking a flexible On-Call RN (PRN) to support patients in the San Angelo area. You will respond to after-hours calls, assess needs, and provide symptom management and end-of-life care in patients’ homes or facilities.

The role requires an active Texas RN license, strong assessment skills, and the ability to work independently while traveling throughout the service area. CPR certification is also expected.

Qualifications

• Active Texas RN license is required.
• Strong assessment and critical thinking skills are essential.
• Excellent communication and customer service skills are preferred.

Responsibilities

• Respond to after-hours calls with professionalism and compassion.
• Assess patient needs and perform appropriate nursing interventions.
• Complete urgent visits in patients' homes or facilities.
• Provide symptom management and end-of-life support.
• Communicate status changes with the clinical team and physicians.
• Educate and support patients, caregivers, and families.
• Document accurately in the EMR system.
