Job summary

Concho Hearts Hospice is seeking a PRN Registered Nurse (RN) to provide compassionate hospice care in Tyler and surrounding areas. The role offers flexible scheduling and the opportunity to work in patients' homes or facilities, delivering comfort and dignity in end-of-life care.

The RN must hold an active Texas license, demonstrate strong assessment and communication skills, and be able to work independently with reliable transportation for local travel.

Responsibilities

• Provide compassionate nursing care to hospice patients in their homes or facilities.
• Complete PRN visits, admissions, or support visits as needed.
• Assess patient conditions and communicate changes to the care team.
• Provide education and support to patients and families.
• Document visits and care using a company-provided tablet.
• Collaborate with the interdisciplinary hospice team.
