Qualifications

• Two years of hospice, homecare, hospital medical-surgical, or oncology nursing preferred.
• Excellent observation, verbal and written communication skills, and problem solving abilities.

Responsibilities

• Respond to after-hours patient census and referrals.
• Provide urgent clinical and psychosocial interventions and pain/symptom management.
• Collaborate with the IDG, physicians, and community providers to ensure continuity of care.
• Document care thoroughly and timely, including changes in pain or symptoms.
