Job description

Responsibilities

The Hospice On‑Call Nurse responds to calls from patients, families, caregivers, and referral sources. The nurse assesses patients, uses critical thinking, and delivers care utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. The nurse effectively interacts with patients, significant others, and other interdisciplinary team members while maintaining professional nursing standards.

Key duties include:

  • Being available by phone or pager from 5PM to 9AM Monday through Friday and for the 24‑hour period beginning at 9AM on Saturdays and Sundays.
  • Identifying and coordinating patient/family care in home, skilled nursing facility or residential care facility settings.
  • Documenting and communicating all calls, interventions, and care‑plan changes with the Case Manager and interdisciplinary team.
  • Performing triage of patient‐related calls and responding appropriately by phone or in person.
  • Implementing changes to the plan of care as needed with support from the hospice medical director and/or attending physician.
  • Assessing hospice needs, collecting data on social, economic, and cultural factors, and documenting patient status and responses to care.
  • Assisting patients and families with empathy, respecting confidentiality and privacy.
  • Coordinating services with contracted vendors, outside agencies, and the interdisciplinary team.
  • Attending patient deaths to provide support, contact the mortuary, advise on disposal of narcotics, and notify the IDT and contracted vendors.
  • Providing bereavement resources to families as appropriate.
  • Participating in the agency’s on‑call rotation and covering continuous care hours when assigned.
Qualifications
  • Graduate of an accredited school of nursing.
  • Valid license as a Registered Nurse (RN).
  • Current Basic Life Support certification.
  • Valid state driver’s license.
  • Hospice or Home Health experience preferred; minimum one year of recent professional nursing experience.
  • Strong interpersonal, communication, and problem‑solving skills.
  • Competent with Microsoft Word, e‑mail, and computer use.
  • Capacity to work independently and cope with emotional stress.
  • Sensitivity to patients with advanced illness and their families.
Additional Requirements

Reliable transportation is required; the nurse must have a valid and current driver’s license and auto insurance. Staff may be required to travel to patient locations, including overnight stays and exposure to various weather conditions. The role may involve lifting up to 50lbs and working under stressful conditions.
