Job summary

Heartland Hospice is seeking an After Hours / On-Call Hospice RN to provide compassionate care during evenings, nights, weekends, and holidays. You will respond to urgent needs, support patients at home or in facilities, and coordinate with the daytime team.

You will report to the Administrator or Patient Care Manager Senior, document visits in EMR, and contribute to quality improvement initiatives. A valid RN license, 1+ year nursing experience, and CPR certification are required.

Qualifications

• Current RN license in the state of employment or eligibility to obtain.
• 1+ year nursing experience.
• Current CPR certification and reliable transportation.

Responsibilities

• Respond to patient needs after hours and on weekends.
• Serve as primary liaison between patients, families, physicians, and the hospice team.
• Document visits and communications accurately (EMR).
• Assess patient and family needs to guide care plans.
• Collaborate with LPNs/LVNs, CNAs, and other support staff.
