Affinity Health Management is seeking an Intake Coordinator to lead the patient intake team and manage referral sources, insurance verification, and authorization processes to ensure smooth admissions.

The role involves collaborating with clinical and marketing teams, coordinating with referral sources, and ensuring compliance with CHAP and regulatory requirements. Strong communication and organizational skills are essential.

Qualifications

• Minimum of three years’ experience in health care, preferably in home care operations.
• Proficient in Google Drive/Sheets/Docs.
• Demonstrates ability to supervise and direct professional and administrative personnel.

Responsabilités

• Directs all daily patient referral and intake operations including oversight of intake policies.
• Enters referral/patient information into electronic medical record system and other applications as needed.
• Coordinates with referral sources, marketing and clinical teams to ensure patient transition into hospice care.
• Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.
• Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
• Establishes and maintains positive working relationships with current and potential referral sources.
• Ensures direct oversight of insurance verification and authorization processes.
• Maintains comprehensive working knowledge of Continuum Care’s contractual relationships and ensures that patients are admitted according to contract provisions.
• Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Continuum Care.

Job Description Summary

Responsible for managing all aspects of the patient intake process including managing the members of the intake team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.

Position Qualifications

• High School Diploma required/ College Degree preferred.
• Proficient in Google Drive/Sheets/Docs preferred.

• Ability to market aggressively and deal tactfully with customers and the community.
• Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices.

• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
