Heart to Heart Hospice seeks a Patient Care Manager - Registered Nurse in Temple, TX to direct and coordinate clinical departments, ensuring quality and safety in patient care.

Ideal candidate has an active RN license in Texas, hospice experience, and strong leadership, organizational, and communication skills. This role requires self-motivation and ability to work within a multidisciplinary team.

Formación

• Current State Registered Nurse License.
• Current State Driver's License, reliable transportation, and provide ongoing valid auto liability insurance.
• Two years of hospice RN experience in a clinical or home health setting.
• Ability to work in a multidisciplinary group and stay self-directed and motivated.
• Strong organizational, people, and problem-solving skills; able to multitask multiple projects.
• Strong critical thinking, organizational, interpersonal, and communication skills.

Responsabilidades

• Direct and coordinate clinical departments and sites, ensuring continuity, quality, and safety of services in line with State and federal regulations (Conditions of Participation).
• Supervise nursing staff and provide direction to ensure plan of care compliance and patient needs assessment across the care continuum.
• Support employee and patient education to enhance knowledge and quality of care; train nursing staff to retain high-quality patient care.
