Heart to Heart Hospice in West Houston, TX 77014 is seeking a Patient Care Manager - Registered Nurse to lead clinical departments, coordinate sites, and ensure continuity, quality, and safety of services. You will supervise nursing staff, align plans of care, and drive adherence to state and federal regulations.

This role requires an active RN license, reliable transportation, excellent communication and problem‑solving skills, and the ability to work in a multidisciplinary team environment

Qualifikationen

• Current State Registered Nurse License.
• Current State Driver’s License, reliable transportation.
• Two years of Hospice RN experience in clinical care or home health.
• Motivated, self-directed, flexible, and able to work in a multidisciplinary group.
• Strong organization, people and problem solving skills; multitask ability.
• Strong critical thinking and communication skills.

Aufgaben

• Direct and coordinate clinical departments and sites; ensure continuity, quality, and safety of services in compliance with regulations.
• Supervise and provide direction to nursing staff to ensure quality and continuity of care.
• Support employee and patient education to enhance knowledge and skills; train nursing staff.
• Provide coaching, development, feedback and annual evaluations to team members.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state, and federal laws and Medicare regulations.
