Job summary

Heart to Heart Hospice in Terre Haute, IN seeks a Patient Care Manager - Registered Nurse to lead clinical departments and sites, ensuring continuity, quality and safety of hospice services. You will supervise nursing staff, support education, coaching and timely documentation, while maintaining regulatory compliance in a multidisciplinary team setting.

The role requires an active RN license and hospice nursing experience, with strong organizational and communication skills to drive

Qualifications

• Current State Registered Nurse License is required.
• Two years of Hospice RN experience in clinical care or home health.
• Must be motivated, self-directed, flexible, and able to work in multidisciplinary groups.
• Strong organization, people, problem solving, and multitasking abilities.
• Strong critical thinking and communication skills.

Responsibilities

• Direct and coordinate clinical departments and sites, ensuring continuity, quality, and safety in line with regulations.
• Supervise nursing staff to ensure Plan of Care adherence and patient needs reassessment.
• Support education for employees and patients to enhance knowledge and patient care quality.
• Provide coaching, development feedback, and timely evaluations to team members.
• Ensure timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state, and federal laws and Medicare regulations.
