Job summary

Heart to Heart Hospice in Texas seeks a compassionate Patient Care Manager - Registered Nurse to oversee clinical departments, ensure quality and safety, and coordinate care across sites.

The role demands an active RN license, hospice experience, strong leadership, and excellent communication. You will mentor staff, drive patient-centered care, and help meet regulatory requirements while supporting patients and families.

Qualifications

• Current State Registered Nurse license is required.
• Current State Driver’s License with reliable transportation and ongoing auto liability insurance.
• Two years of experience as a Hospice Registered Nurse in a clinical care setting or home health.
• Must be highly motivated, self-directed, flexible, and able to work in a multidisciplinary group setting.
• Strong organization, people and problem solving skills with the ability to multitask many projects.

Responsibilities

• Direct and coordinate clinical departments and sites; ensure continuity, quality, and safety of services in compliance with state and federal regulations (Conditions of Participation).
• Supervise nursing staff and provide direction to ensure quality, adherence to Plan of Care, and appropriate reassessment of patient needs.
• Support employee and patient education to enhance knowledge and skills; continuously trains nursing staff to retain employees with high quality care.
• Provide coaching, development, feedback and annual evaluations to team members.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure overall compliance with local, state and federal laws and Medicare regulations.
