Heart to Heart Hospice in Texas is seeking a Patient Care Manager - Registered Nurse to lead clinical teams and oversee patient care across hospice services.

The role requires an active RN license, state driving privileges, and at least two years of hospice nursing experience, with strong leadership and communication skills to coordinate care plans and ensure quality outcomes for patients and families served.

Qualifications

• Current State Registered Nurse license.
• Current State Driver’s License with reliable transportation and evidence of valid auto liability insurance.
• Two years of hospice RN experience in a clinical care setting or home health.
• Highly motivated, self-directed, flexible, and able to work in a multidisciplinary group.

Responsibilities

• Direct and coordinate clinical departments and sites, ensuring continuity, quality, and safety in line with regulatory requirements.
• Supervise nursing staff and guide team to meet care plans and reassess patient needs.
• Support employee and patient education; train nursing staff to retain high-quality care.
• Provide coaching, development, feedback, and annual evaluations to team members.
• Ensure timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state, and federal laws, Medicare regulations, and policies.
