Job summary

AccentCare, Inc. in Fall River, MA is seeking a full-time, after-hours Hospice Admission RN to join a compassionate care team.

You will perform comprehensive nursing assessments, coordinate admissions, and collaborate with physicians, IDG, and families to develop and execute individualized plans of care in compliance with regulatory requirements. The role requires active RN license in the state, an associate degree (preferred), and willingness to participate in on-call rotations.

Qualifications

• RN license in the applicable state is required; associate degree preferred.
• CPR certification recommended (required for direct patient care in some states).
• Experience in hospice, home health, medical-surgical, or geriatrics/pediatrics preferred.

Responsibilities

• Conduct comprehensive nursing assessments and collaborate with patients, families, physicians, and IDG to develop individualized care plans.
• Maintain timely, accurate documentation and update care plans as patient status changes.
• Educate and provide emotional support to patients and caregivers; participate in on-call rotations, admissions, and care transitions.
