AccentCare is seeking a dedicated RN for after-hours admissions in the Fall River, MA area. Full-time, evenings through early mornings, with on-call rotation and patient-focused care.

Qualifications include RN licensure, associate degree preferred, CPR recommended, and experience in hospice or home health. Proficiency with EMR and Microsoft Office is required. Benefits and a supportive team culture are offered.

Qualifications

• Current licensure as a Registered Nurse in the applicable state is required, with an associate degree preferred.
• Must participate in an on-call rotation and maintain reliable transportation.
• Strong communication and organizational skills; proficiency in Microsoft Office Suite and EMR systems.
• This role involves lifting patients and moving supplies as part of care delivery.

Responsabilités

• Conducts comprehensive nursing assessments and collaborates with patients, families, physicians and the IDG to develop and evaluate individualized plans of care.
• Maintains timely documentation and coordinates care with the team to optimize resource use.
• Provides education and emotional support to patients and caregivers, participates in on-call rotations and care transitions.
• Coordinates care as Care Manager or On-Call, including scheduling, medication management and IDG collaboration.
