Job summary

Visiting Nurse Association of Ohio is expanding its PRN Hospice NP team in the Cleveland / Northeast Ohio area. This role focuses on face-to-face hospice eligibility visits, comprehensive home assessments, and collaborative care planning to ensure high-quality end-of-life care.

You will manage medications for comfort and symptom control and support family discussions. Minimum 2 years of APRN experience with an active Ohio NP license and DEA registration is required.

Qualifications

• Active NP license (Ohio) + BLS certification.
• Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP).
• Active DEA registration.
• Reliable vehicle and valid driver's license with ability to travel throughout the service area.
• Minimum 2 years of advanced practice nursing experience.
• Familiarity with Medicare and Medicaid hospice regulations.

Responsibilities

• Conduct face-to-face hospice eligibility visits and complete required documentation in compliance with Medicare hospice regulations.
• Perform comprehensive assessments of hospice patients in their homes, including symptom management evaluation and goals-of-care discussions.
• Collaborate with the attending physician and interdisciplinary hospice team to develop and adjust individualized plans of care.
• Prescribe and manage medications focused on comfort, pain management, and symptom control.
• Provide advanced clinical guidance to hospice field staff including RNs, LPNs, and aides.
• Educate patients and families on disease progression, comfort-focused care, and end-of-life planning.
• Participate in interdisciplinary team meetings and case conferences as needed.
• Maintain accurate and timely documentation in the EMR within required timeframes.
• Ensure compliance with Medicare, Medicaid, and CHAP hospice standards.

Pay Rates – Per Completed Visit

• Per Visit - $90
• Downtime (pre-approved) - $60/hour

"Completed" = visit done in the home + documentation finished within 24 hours + orders/signatures uploaded + meets QA standards.

What You'll Do

Requirements

Must-Have

• Active NP license (Ohio) + current BLS certification

Preferred / Nice-to-Have

• Prior hospice or palliative care NP experience
• ACHPN certification or willingness to pursue
• Experience conducting face-to-face hospice eligibility visits
• Familiarity with HCHB or similar home health/hospice EMR
• Strong goals-of-care conversation and family communication skills

Salary: $60 - $90 per visit
