Pay Rates - Per Completed Visit

• SOC / ROC - $150
• Eval (complex skill add‑on) - $115
• Revisit / Discharge - $70
• Downtime (pre‑approved wait time) - $45/hr

Premium Add‑ons

• Same‑Day / Urgent: +$25
• Weekend: +$25
• Holiday: +$50

"Completed" = visit done in the home + documentation finished in EMR within 24 hours + orders/signatures uploaded + meets QA standards.

Requirements

Must‑Have

• Active PT license (Ohio) + current BLS certification
• Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
• Minimum 1 year of clinical physical therapy experience

Preferred / Nice‑to‑Have

• OASIS certification or demonstrated OASIS experience
• Prior home health experience
• Familiarity with HCHB or similar home health EMR

Salary: $45 - $115 per visit
