Job summary

The Visiting Nurse Association of Ohio Home Health is seeking a Physical Therapist to join their PRN team. This role offers the flexibility to set your own availability and provides competitive pay rates per completed visit. Must have an active PT license in Ohio and a minimum of one year of clinical experience. The role includes the provision of therapy services within patients' homes, and documentation must be completed in accordance with QA standards. Mileage reimbursement is also offered.

Responsibilities

• Provide therapy services in patients' homes as per schedule.

Pay Rates – Per Completed Visit

SOC / ROC - $150

Eval (complex skill add-on) - $115

Revisit / Discharge - $70

Downtime (pre-approved wait time) - $45/hr

Premium add-ons:

• Same-Day / Urgent: +$25
• Weekend: +$25
• Holiday: +$50

"Completed" = visit done in the home + documentation finished in EMR within 24 hours + orders/signatures uploaded + meets QA standards.

Cleveland / Northeast Ohio | Weekdays, weekends, evenings, on-demand - you set your availability.

Requirements

Must-Have:

• Active PT license (Ohio) + current BLS certification
• Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
• Minimum 1 year of clinical physical therapy experience

Preferred / Nice-to-Have:

• OASIS certification or demonstrated OASIS experience
• Prior home health experience
• Familiarity with HCHB or similar home health EMR
