Job summary

VNA of Ohio – Hospice in Cleveland / Northeast Ohio is seeking a Hospice Nurse Practitioner for a PRN, per-visit position. Earn $90 per completed visit with downtime paid at $60/hour. Flexible scheduling with limited weekend/holiday requirements.

The role supports advanced clinical care, family education, and documentation compliance, working closely with the hospice team to manage patient comfort and goals-of-care.

Qualifications

• Active NP license (Ohio) and current BLS certification.
• MSN or DNP degree.
• Active DEA registration.
• Reliable vehicle and valid driver's license for travel in Cleveland / Northeast Ohio.
• Minimum 2 years of advanced practice nursing experience.
• Familiarity with Medicare and Medicaid hospice regulations.

Responsibilities

• Conduct face-to-face hospice eligibility visits and complete required documentation per Medicare hospice regulations.
• Perform comprehensive home-based assessments with symptom management and goals-of-care discussions.
• Collaborate with attending physician and hospice team to develop and adjust plans of care.
• Prescribe and manage medications focused on comfort and symptom control.
• Provide clinical guidance to hospice staff including RNs, LPNs, and aides.
• Educate patients and families on disease progression and end-of-life planning.
• Participate in interdisciplinary team meetings and case conferences as needed.
• Maintain accurate EMR documentation within required timeframes.
• Ensure compliance with Medicare, Medicaid, and CHAP standards.

Pay Rates – Per Completed Visit

Per Visit - $90

Downtime (pre-approved) - $60/hour

\"Completed\" = visit done in the home + documentation finished within 24 hours + orders/signatures uploaded + meets QA standards.

What You'll Do

• Conduct face-to-face hospice eligibility visits and complete required documentation in compliance with Medicare hospice regulations
• Perform comprehensive assessments of hospice patients in their homes, including symptom management evaluation and goals-of-care discussions
• Collaborate with the attending physician and interdisciplinary hospice team to develop and adjust individualized plans of care
• Prescribe and manage medications focused on comfort, pain management, and symptom control
• Provide advanced clinical guidance to hospice field staff including RNs, LPNs, and aides
• Educate patients and families on disease progression, comfort-focused care, and end-of-life planning
• Participate in interdisciplinary team meetings and case conferences as needed
• Maintain accurate and timely documentation in the EMR within required timeframes
• Ensure compliance with Medicare, Medicaid, and CHAP hospice standards

Requirements

Must-Have:

• Active NP license (Ohio) + current BLS certification
• Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Active DEA registration
• Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
• Minimum 2 years of advanced practice nursing experience
• Familiarity with Medicare and Medicaid hospice regulations

Preferred / Nice-to-Have:

• Prior hospice or palliative care NP experience
• ACHPN certification or willingness to pursueExperience conducting face-to-face hospice eligibility visits
• Familiarity with HCHB or similar home health/hospice EMR
• Strong goals-of-care conversation and family communication skills
