Job summary

VNA of Ohio – Hospice is seeking a Hospice Nurse Practitioner to join our PRN team serving the Cleveland and Northeast Ohio area. You will conduct eligibility visits, perform comprehensive home assessments, and collaborate with a multidisciplinary team to develop plans of care focused on comfort and symptom relief.

The role requires an Ohio NP license, MSN or DNP, DEA, and a valid driver’s license with willingness to travel.

Qualifications

• Active NP license (Ohio) and current BLS certification.
• Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP).
• Active DEA registration.
• Reliable vehicle and valid driver's license with ability to travel throughout the service area.
• Minimum 2 years of advanced practice nursing experience; familiarity with Medicare and Medicaid hospice regulations.

Responsibilities

• Conduct face-to-face hospice eligibility visits and complete required documentation in compliance with Medicare hospice regulations.
• Perform comprehensive assessments of hospice patients in their homes, including symptom management evaluation and goals-of-care discussions.
• Collaborate with the attending physician and interdisciplinary hospice team to develop and adjust individualized plans of care.
• Prescribe and manage medications focused on comfort, pain management, and symptom control.
• Provide advanced clinical guidance to hospice field staff including RNs, LPNs, and aides.
• Educate patients and families on disease progression, comfort-focused care, and end-of-life planning.
• Participate in interdisciplinary team meetings and case conferences as needed.
• Maintain accurate and timely documentation in the EMR within required timeframes.
• Ensure compliance with Medicare, Medicaid, and CHAP hospice standards.
