Job summary

Ohio Living in Cleveland seeks a Nurse Practitioner to provide preventive medical, palliative, and educational services to patients across home health and hospice programs. You will comply with state and local standards using our comprehensive clinical-financial computer system and EMR software to deliver high‑quality care with integrity and compassion.

You will assess health histories, manage illnesses, prescribe appropriate therapies, coordinate referrals, and contribute to quality improvement

Qualifications

• Current unencumbered Ohio CNP license.
• Current prescriptive authority certificate or externship certificate to prescribe.
• Minimum two years of experience in home health and/or hospice.
• Proficiency with Windows, Microsoft Office, and the internet.
• Must read/write/speak English proficiently.

Responsibilities

• Conducts comprehensive medical and social histories and performs physical examinations.
• Treats common acute illnesses, chronic and stable health conditions.
• Orders and interprets lab tests and x-rays.
• Provides information, instruction, and counseling on health maintenance and disease management.
• Refer patients to other health professionals as needed.
• Communicates with staff, patients and families to promote optimal outcomes.
• Completes documentation accurately and timely per regulations.
• Participates in quality improvement projects.
• Commitment to excellent customer service for internal and external customers.

Essential Activities And Tasks

Care Management - 100%

• Conducts comprehensive medical and social histories and performs physical examination of healthy, acutely ill, chronically ill patients or terminal patients.
• Treats common acute illnesses, chronic and stable health conditions through prescribing medications and other treatments.
• Orders and interprets lab tests and x-rays.
• Provides information, instruction, and counseling on health maintenance, health promotion, social problems, illness prevention, illness management, palliative care and medication use.
• Refers patients to other health professionals as needed.
• Communicates with staff members, patients and families to promote optimal patient outcomes.
• Completes documentation accurately and timely while maintaining state, federal and agency regulation.
• Participates in quality improvement projects.
• Commitment to excellent customer service for internal and external customers.

Qualifications

Education

• Current unencumbered license for the state of Ohio to practice as a Certified Nurse Practitioner (CNP) required.
• Current Certificate of Prescriptive Authority or Externship Certificate to Prescribe.

Experience

• Two years experience in the home health and/or hospice field required.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.
• Proficiency with electronic medical records (EMR) preferred.

Other Requirements

• Must be able to read, write, speak, and understand the English language.
• Must possess a valid driver's license and acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.
