Job summary

BAYADA Hospice is seeking a compassionate Nurse Practitioner to join our in-home hospice team in Tucson, Arizona. This NP-per diem role provides essential medical oversight and palliative care to patients at home, collaborating with a multidisciplinary team.

Candidates should have AZ NP licensure and at least one year of clinical experience; hospice or home health exposure is preferred. Strong communication and willingness to travel are required, with comprehensive BAYADA benefits available.

Qualifications

Licensed as a Nurse Practitioner in AZ.
At least 1 year of clinical (medical/surgical) experience.
Home health, hospice or palliative care experience preferred but not required.
Experience with electronic medical records (EMR) preferred.
Ability to travel to assigned cases.
Strong interpersonal and communication skills.

Responsibilities

Perform home-based hospice NP visits and recertifications in designated geographic areas.
Monitor patient conditions and report changes to the Clinical or Client Services Manager.
Follow up on orders and document care accurately.
Conduct patient assessments as needed.
