BAYADA Hospice is seeking a compassionate Nurse Practitioner to join our in-home hospice team in Tucson, Arizona. This NP-per diem role provides essential medical oversight and palliative care to patients at home, collaborating with a multidisciplinary team.

Candidates should have AZ NP licensure and at least one year of clinical experience; hospice or home health exposure is preferred. Strong communication and willingness to travel are required, with comprehensive BAYADA benefits available.

BAYADA Hospice is hiring a compassionate and dedicated Per Diem Nurse Practitioner, (NP) to join our team. The coverage area for this position is in Tucson, Arizona.

Prior home care, hospice, or palliative care experience strongly preferred, but not required. As a hospice nurse NP, you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Nurse Practitioner (NP) Responsibilities:

• Making home face to face hospice recertification visits to clients in designated geographic territories.
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.

Qualifications for a Nurse Practitioner (NP):

• A current license as a NP in AZ
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care, hospice and/or palliative care experience strongly preferred, but not required.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
• Strong interpersonal skills.
